Move inpage-provider and port-stream outside

With the creation of the [metamask-extension-provider](https://github.com/MetaMask/metamask-extension-provider ) we have our first non-core module that is dependent on the inpage-provider and port-stream.
To reduce the size of its dependencies, I have moved the
[metamask-inpage-provider](https://github.com/MetaMask/metamask-inpage-provider ) into its own module, as well as [extension-port-stream](https://github.com/MetaMask/extension-port-stream ).
This allows them to be more easily depended & iterated on by external
projects.

According to the current implementation any domain ending in .eth or .test will try to resolve through ENS even if the DNS is able to resolve it.
changing `onBeforeRequest` to `onErrorOccurred` will only resolve those .eth or .test domains that the DNS is not able to resolve.

In [this reddit post](https://www.reddit.com/r/Metamask/comments/8r3nsu/help_me_please_somebody_stole_my_ethers/ ) a user suggests they got some ether stolen after visiting IDEX. Their ether was sent to [this address](https://etherscan.io/address/0x9bcb0a9d99d815bb87ee3191b1399b1bcc46dc77 ), which is full of comments of people telling similar stories of being phished on IDEX. I think we can safely block this, and probably safe some people some money.

Fixes #4462
Ensures that resetAccount() can work on non-stock providers.
I'm unclear how this was ever working, this code hasn't moved in months,
but users report it recently breaking. Maybe we only recently pushed it
to prod.

On password submission, this change passes known accounts to the
preferencesController (responsible for nickname management), and removes
unknown entries.

When first initializing, recent-block controller now back-fills up to
its desired history length. This makes estimated gas prices reflect a
longer recent history, even when first switching to a new network.

...
EthStore was only being used for tracking account balances and nonces now, so I removed its block-tracking duties, renamed it account-tracker, and removed it as a dependency from `KeyringController`, so that KRC can go live on without a hard dep on it.

Do not mark a retry tx failed that has been broadcast successfully
...
Fixes #2115
If a tx has been braodcast, the only failures we should accept are:
- Never mined
- On chain failure
We had a section of code that would mark a tx failed during any unknown error during a retry.
Now no retry > 1 will ever mark a tx failed, since it has been broadcast, and may be mined.

Create distinct responsive 'home.html' file, hard-code popup.html size
...
Because firefox was having inconsistent sizing, made a second html file for forcing the view to a certain size. Still allows us to develop a responsive interface via the `home.html` file, which shares all the same react JS & CSS as popup.html.
